Steps to Follow to Board
Unaccompanied Minors
on Breeze Bus

  • Review and agree to abide by Breeze Minor Policy.
  • Identify your child’s age range and click on the corresponding “+” sign.
  • Download the correct form by clicking the orange button for your child’s age.
  • Print the downloaded form. (If you can’t print at home, the bus driver will have blank forms.)
  • Fill out all required sections of the form.
  • Give the completed form to the bus driver.
  • Please note: An OPT IN minor form has to be presented to the driver by the responsible adult at drop off and pickup. An OPT OUT minor form has to be filled out by the responsible adult but can be presented to the driver by the minor.
  • Please note: Children without a completed paper form will not be permitted to board the bus.

All minors traveling without an adult must present a completely filled-out Minor Form.

Note: By signing tHE OPT IN OR OPT OUT form, you acknowledge that you have read, understood, and agree to abide by the Central Oregon Breeze Minor Policy.

 

OPT IN: Ages 7-17 (MINOR DOES NOT HAVE A GOVERNMENT ISSUED ID)
  • This OPT IN form must be completely filled out by a responsible adult  and handed to the driver by the responsible parent/guardian at boarding.
  • A designated responsible adult has to check the minor in with the bus driver AT DROP OFF AND PICKUP. These two designated responsible adults have to be listed on the minor form.
  • Both designated responsible adults, at drop-off and pick-up,  must show their driver’s ID and sign the minor form in front of the driver before the minor gets on and off the bus. These ID numbers have to match the filled out minor form. There are no exceptions to this rule. 
  • The OPT IN/MINOR will be given a Lanyard with ID Tag to always wear and keep visible during the trip, provided by the driver. 
  • The OPT IN/MINOR must sit in the first 3 rows of the bus (no exceptions).
  • The OPT IN/MINOR must remain seated until/unless the driver releases them to an adult or to access bathroom facilities during the trip.
  • Once the designated responsible adult signs for the minor and the driver has collected their Lanyard-ID, then and only then will release the minor to the designated adult.

Pickup Guidelines 

  • Responsible adults and Minors refusing to follow these policies may be asked to not ride again.
  • Please be early when picking up your OPT IN Minor. If the designated responsible adult is 5 minutes late or more, from the scheduled pick-up time time; there is an additional $20.00/hour sitting fee (Payable in cash, at the time you pick up the minor).  
  • After waiting 5 minutes past the scheduled drop off time, the Driver will continue to the next stop with the minor remaining on board in the care of the driver. 
  • It is the responsibility of the designated responsible adult to arrange to meet the driver at the next stop(s) to collect the OPT IN Minor and make payment. 
  • The bus will not hold additional time at the next stop. Please understand that the bus is following a timed schedule. Being late at one stop will delay the route and impact all passengers on the bus.

Courtesy and Safety  

  • For your child’s safety please instruct your child to always stay seated in their seat.
  • Out of respect for other passengers on the bus please keep activities and speaking at quiet levels.  
  • Music and game players need to have headsets and not disturb other passengers. 
  • Please instruct your child not to disturb the driver while they are operating the bus.  If your child needs to ask questions, please have them ask the driver during stops/breaks. 
  • There are two restroom breaks, instruct your child not to wander and be timely. The bus driver will make sure your child gets back on the bus. 
  • Send enough activities, snacks, water, etc. to keep your child well cared for and entertained quietly.  Remind them to put all trash in the proper place. 

Minors that are disruptive or incorrigible may be removed from the bus and/or banned from riding in the future. A Parent/Guardian or responsible adult will have to meet the bus early or in EXTREME cases the child will be delivered to the custody of Law Enforcement for their safety.

OPT OUT: Ages 15-17 (MINOR HAS A GOVERNMENT ISSUED ID)
    • This OPT OUT form must be completely filled out by the responsible adult and handed to the driver at boarding.
    • The responsible adult does not have to be present at drop-off or pick-up but has to fill out the form to establish their legal authority on behalf of the child.
    • The OPT OUT MINOR must show a valid Government Issued ID to the Bus Driver.
    • Example: Government issued driver’s license or permit, state issued ID card, or passport
    • Not accepted: School ID, credit cards, or other non-government issued IDs
    • OPT OUT Minors may leave the bus at a different location than what is listed on their reservation.
    • OPT OUT Minors will be considered completely responsible to make their own decisions and are responsible 100% for themselves and any minors in their care.
    • An OPT OUT Minor may supervise up to two OPT IN Minors, granted full responsibility for the OPT IN minor(s). If the OPT OUT Minor disembarks the bus at a different location than indicated on their ticket, they must bring the OPT IN minors with them.
    • If the responsible adult does not agree with the OPT OUT policy, please book an OPT IN Minor ticket for an extra level of supervision.
